The precision analysis showed an SD of 0.5 (95% CI for SD, 0.3â0.9) for the sample within the reference range, with a coefficient of variation (CV) of 4.7% (mean, 10 mm/h); and, for the sample outside the reference range, an SD of 2.5 (95% CI for SD, 1.7â4.5) and a CV of 3.6% (mean, 68.6 mm/h). The testing was performed at the UH Lab by 2 resident pathologists (B.V. and L.D.L.C.). Because our laboratory is located 1 mile (5,280 feet) above sea level (Albuquerque, NM), the ESR of our patient population is expected to be lower because of reduced oxygen tension and resultant polycythemia, excluding high altitude as a cause of the systematic bias. We investigated various potential factors that might be responsible for systematic bias.
After 30 minutes, the measured ESR was automatically printed out by the instrument. We compared the Streck ESR-Auto Plus instrument for ESR measurement with the Sediplast Westergren method as the reference method and demonstrated that the 2 methods are correlated at 95%. The same study also proved the stability of refrigerated EDTA blood samples for 24 hours.8 Because we used manual transfer of EDTA blood samples into the ESR vacuum tubes for method comparison, we further validated the stability of 5 EDTA blood samples by measuring the ESR using the Streck method immediately after blood draw and after storing samples between 2°C and 10°C for 24 hours.
We modified our reference ranges to rectify the systematic bias found during validation. Miao7 in China studied the effect of geographic variables on ESR and showed that the ESR was inversely proportional to the altitude. To evaluate the effect of sample temperature on ESR measurement, we measured the ESR using the Streck method on 2 EDTA blood samples drawn from a healthy volunteer immediately after the blood draw at 37°C, after allowing the samples to normalize to room temperature for 1 hour, after storing them at 2°C to 10°C for 8 hours, followed by testing them with and without normalization to room temperature. The test for measuring the ESR was first described by Westergren and Fahraeus and is well known as the Westergren method.1 There have not been many changes to the originally described method,and all subsequently introduced methods have been evaluated against the Westergren method as the standard.
